To qualify for OHIP-covered MedsCheck services, you must meet the following requirements:
- Taking 3 or more prescription medications for chronic conditions
- Ontario resident with valid OHIP card
- Haven't had a MedsCheck in the past 12 months
- Referred by physician or self-referred
